The Diabetes Coordinator will be responsible for the day-to-day operation of the diabetes program, working with the agency’s pharmacy to provide diabetic medication to clients in need of assistance.

The Coordinator will work closely with the client to coordinate and provide care while improving health outcomes through education and additional resources.  Methods suggested, but not limited to, are phone conversation with patient, face to face visits, A1C challenge, on-site educational events, follow-up correspondence, etc.  The Coordinator will track progress of patients. This person will insure that the national standards for privacy are met and maintained at all times.


With the assistance of students of Wayne State University Applebaum School of Pharmacy, the coordinator organizes the A1C challenge, support groups, nutritional and exercise classes.


The position is fulltime (hourly) and reports to the Local Program Manager


Qualifications required:

Education:  Bachelor’s degree in related healthcare field

2 years’ experience as a medical case manager, preferably working with diabetic patients.

Excellent knowledge of case management principles

Knowledge of medical terminology

Excellent interpersonal skills and counseling skills

Excellent organizational and project management skills

Problem solving skills and ability to multi-task

Excellent working knowledge of Microsoft suite.
